After you've determined the proper category(s) for your hours per Sec IV, submit for credit using the form below. For example: Ushering and/or PREP involvement for 20 hrs would be reported as 4a (church activity) under Sec IV. You only have to be concerned with the section IV designations for reporting hours/visits..
To report the above example it would be 4a/20 hrs.
If you prefer to keep track of your activities and want to submit your totals at the end of the year, make a copy of the form (s) below and submit to the Financial Secretary in January of the next year.
